When you’re ready to move forward, please follow these steps:
- Register on our Patient Portal using the button below.
- For families, each family member, including children, must register separately; shared Portal access can be established after accounts have been created.
- After confirming your account, sign into the Patient Portal and go to 'Forms & Questionnaires' in the right-hand sidebar.
- Under 'Onboarding - Step 1', please fill out and submit the Initial Patient Questionnaire.
- If you're answering on behalf of your family, please fill out a separate questionnaire for each family member.
- We will review your responses within 48 hours and match you with up to 3 Lyme-literate providers for free.
- Choose the provider who best fits your needs and we’ll guide you from there.
