When you’re ready to move forward, please follow these steps:
- Register through our Patient Portal using the button below.
- For families, each family member, including children, must register separately; shared Portal access can be established after accounts have been created.
- Once your registration is complete, please sign into the Patient Portal and go to 'Forms & Questionnaires' in the right-hand sidebar.
- Under 'Onboarding - Step 1', please fill out both the Initial Patient Questionnaire and the Readiness Assessment and submit them.
- If you're answering on behalf of your family, please fill out separate questionnaires for each family member.
- We will review your responses with our Chief Medical Officer and match you with up to 3 Lyme-literate providers via e-mail for free. (Please note matches are not guaranteed, they will depend on provider availability, if we can meet your needs, etc.)
- Select the one who fits your personal situation best and we’ll provide you next steps from there. (Please note you're under no obligation to move forward after being matched to a provider.)
